The pattern is simple: inhale for four counts, hold for four, exhale for four, hold for four. Repeated for four to eight cycles, it produces a rapid shift in nervous system state — reducing anxiety, sharpening focus, and creating a sense of calm control that is distinct from relaxation.
The holds are key. Breath retention, even brief, increases carbon dioxide tolerance over time — training the body to remain calm at higher levels of physiological arousal. This is why the technique is used in high-stakes environments where clear thinking under pressure is essential.
Research has confirmed significant effects on anxiety, attention, and stress response. A 2017 study in the Journal of Neurological Sciences found structured breathing practices including box breathing produced measurable improvements in autonomic nervous system function after just a few weeks of consistent practice.

Coherent Breathing — The Resonance Frequency
Of all the breathwork techniques supported by physiological research, coherent breathing may be the most precisely calibrated. Developed and studied extensively by clinical psychologist Richard Brown and psychiatrist Patricia Gerbarg, it involves breathing at a rate of five breaths per minute — inhaling for six seconds, exhaling for six seconds, in a slow, even rhythm.

At the more intensive end of the spectrum sits a family of activating breathwork practices — faster, fuller breathing patterns that deliberately alter blood chemistry and can produce profound shifts in consciousness, emotional release, and psychological insight.
The most clinically studied of these is Holotropic Breathwork, developed by psychiatrist Stanislav Grof in the 1970s as a non-pharmacological tool for accessing non-ordinary states of consciousness. The Long Game: How Breathwork Changes You Over Time
The acute effects of breathwork — the immediate calm after box breathing, the clarity after coherent breathing, the emotional release after a holotropic session — are real and valuable. But they are not the full story.
The deeper promise of consistent breathwork practice lies in what researchers call respiratory retraining — the gradual shift of your default, resting breathing pattern toward something slower, deeper, more nasal, and more diaphragmatic. This shift does not happen overnight. It happens across weeks and months of consistent practice, as new breathing habits begin to overwrite the dysfunctional patterns laid down by years of chronic stress.
When this shift takes hold, the changes are not confined to the moments when you are consciously practising. Your baseline heart rate variability improves. Your cortisol rhythm normalises. Your sleep deepens. Your threshold for stress activation rises. You begin to recover from difficult moments faster — not because you are suppressing your response, but because your nervous system has become genuinely more resilient.
